Essential Plan

Essential Plan

The Essential Plan through NY State of Health is offered by UnitedHealthcare Community Plan. It's for lower-income individuals in New York who meet the income or citizenship status requirements and who are not eligible for Medicaid or other coverage.

There is no monthly premium for Essential Plan coverage. All Essential Plan enrollees will also have dental and vision benefits with no cost sharing for those covered services.

Medicaid

UnitedHealthcare Community Plan

The New York State Medicaid Managed Care Plan is offered through UnitedHealthcare Community Plan. It’s for New York State residents who meet the income or disability requirements.

UnitedHealthcare Wellness4Me

This plan is for New York residents age 21 and who qualify for Medicaid coverage.

The Wellness4me Plan is offered through UnitedHealthcare Community Plan. Wellness4Me is a Health and Recovery Plan, or HARP, approved by New York State. HARPs are a new kind of plan that provide Medicaid members with their health care, plus care for behavioral health, including Serious Mental Illness and Substance Use Disorders.

Dual Special Needs plans

Dual Special Needs plans (also called Dual Health plans or D-SNPs for short) are for people who have both Medicaid and Medicare. Dual plans cover most doctor visits, hospital stays and prescription drugs. They offer many extra benefits beyond those you get with either Original Medicare or Medicaid.

Children's Health Insurance Program

Child Health Plus

The New York State Child Health Plus program from UnitedHealthcare Community Plan is for children up to age 19. Members must be residents of New York and are not eligible for Medicaid or enrolled in or access to NYSHIP benefits. You can choose your child’s doctor. And your child gets many services to stay healthy.

New York Essential Plan: 

  • Low-income individuals with families
  • Ages 19-64
  • Doesn't qualify for Medicaid or Child Health Plus

New York Medicaid Plans:

  • Pregnant women and children
  • Low-income individuals with families

Enrollment:

The New York State Medicaid Managed Care plan and the Essential Plan are offered through UnitedHealthcare. 

Enrollment is open all year if you are eligible for Medicaid, Child Health Plus and the Essential Plan.

OTC benefits

The Essential Plan offered by UnitedHealthcare provides a $100 prepaid card to help you save on healthy food and over-the-counter (OTC) items:

  • Use the card in 55,000+ retail stores for food like fruits, veggies, meat, seafood, dairy, water and more
  • Pick from many OTC items like toothpaste, first aid kits, cough drops, bladder control pads, pain relievers, cold remedies and more

The Healthy First Steps® team is here for you

  • Have pregnancy questions? We’ve got answers
  • Looking for a doctor or nurse midwife? We'll help find one near you
  • Need rides to and from your prenatal checkups? We can set them up
  • Want help with community services like Women, Infants and Children (WIC)? We can assist you

Vision, Dental and Hearing care

Make sure your sight, teeth, speech and hearing are at their best. Benefits include:

  • Vision. Covers eye exams and glasses
  • Dental. We cover cleanings, checkups and dental work
  • Hearing. Screenings and therapies, if needed, are provided
